As the situation and details regarding the coronavirus and its impact continue to unfold and change, we want to make sure you're aware of various benefit-related resources to help you and your family.

Health Care Resources

Virtual visits with your physician or telemedicine options are available so you can receive medical or behavioral health help while minimizing exposure to potentially contagious viruses. You may use these services for ALL medical needs, not just those related to the coronavirus. More information related to virtual visits and telemedicine options are listed by employee type and insurance plan below.

Insurance Reimbursement for At-Home COVID Tests: Effective January 15, 2022, through the end of the public health emergency, the federal government issued new requirements for reimbursement of at-home COVID-19 tests. Reimbursement information based on your employee type and insurance plan is available below. Additionally, the federal government is giving each family 4 free COVID tests regardless of insurance type. 

 

Employees covered by Blue Cross Blue Shield of Michigan (BCBSM) Community Blue, Blue Care Network (BCN), or BCBSM Consumer Driven Health Plan (CDHP)

Virtual visits scheduled directly with your physician are subject to regular copayment and deductibles as they would have applied going into the office.

Reimbursement of up to 8 at-home COVID-19 tests per member is available every 30 days. This is the number of total tests. If purchasing a kit with 2 tests, the kit would be considered reimbursement for 2 of the 8 allowed tests.

  • May be obtained for free in-person at a participating in-network CVS pharmacy. NOTE: Pharmacies have the option to opt-out of the program. Please ask the pharmacy if they are participating in the program prior to purchasing.
  • You may purchase a COVID test and then provide a receipt for reimbursement. Find instructions and more info on the CVS/Caremark website.
  • The tests must be FDA Authorized. Qualifying tests include over-the-counter antigen and molecular tests.

  • Tests submitted for reimbursement under your health plan should not be paid for or submitted as an eligible expense to a Health Care Flexible Spending Account or Health Savings Account.

International employees covered by Cigna Global Health Advantage (CGHA)

Virtual visits scheduled directly with your physician are subject to regular copayment and deductibles as they would have applied going into the office.

  • Tests much be purchased and delivered within the U.S.
  • In order to be reimbursed, you should pay for the tests at the time of purchase and then submit a claim to Cigna that includes the following:
    • Covid OTC At Home Claim Form – this form is available in the Cigna Envoy Toolkit under Documents > Your Claim Form.
    • Must include attestation stating the test is not for employment purposes, has and will not be reimbursed by another source and is not for resale.
    • Purchase receipt including date of purchase, price and clearly detailing the purchase of the approved at-home COVID test document.
    • Claims can be submitted via the options on the form:
      • Cigna Envoy online claims submission
      • By mail using the address on the back of the Cigna ID card
      • Fax to 800-243-6698 or 302-797-3150
  • The tests must be FDA Authorized. Qualifying tests include over-the-counter antigen and molecular tests.

  • Tests purchased between Jan. 15 – Jan. 31, 2022 from a non-preferred pharmacy can receive reimbursement by submitting this form.

Retirees covered by the Humana MSU Non-Medicare and Humana Group Medicare Advantage PPO plans.

Medical visits covered through Doctor on Demand with $20 copayment. Behavioral Health visits covered through Doctor on Demand with $0 copayment.

Retirees covered by Humana Group Medicare Advantage Plan are covered through MDLive when the physician is in-network. Non-specialist telehealth visits are covered with $0 copayment and specialist telehealth visits are subject to a 4% co-insurance. For information contact MDLive at 1-888-673-1992 (TTY: 711).

MSU Humana Non-Medicare Members:

MSU non-Medicare members enrolled in Humana will have two options to get COVID-19 test kits at no-cost.

Option 1: Get reimbursed for COVID tests purchased on or after Jan. 15, 2022:

  1. Purchase your test kit and keep your itemized receipt
  2. Complete all information requested on the Health Benefits Claim Form
  3. Mail the completed form with the original itemized receipt to the address on the back of your member ID card as soon as possible
  4. It can take up to 30 days to process your claim, and the reimbursement will be sent as a check to the home address Humana has on file
  5. Note that reimbursement will include the test kit only. No shipping fees will be covered by Humana.

Option 2: Purchase at-home, over-the-counter test kits at a network pharmacy counter with your Humana ID card on or after January 15, 2022:

  1. Take the kit(s) to the pharmacy counter for check-out
  2. You will be asked to provide your Humana ID card
  3. The pharmacy staff will process your purchase and receive payment directly from Humana. You will owe zero dollars for up to 8 test kits, per member, per month

MSU Humana Medicare Members:

This program does not include reimbursement for tests for those enrolled in Medicare. The Department of Health and Human Services is providing up to 50 million free, at-home tests to community health centers and Medicare-certified health clinics for distribution at no cost to patients and community members.
