Tutorial: How to Complete the Health Plan Affidavit for Spouse/Other Eligible Individual (OEI)

PLEASE NOTE:
If you currently cover a spouse or other eligible individual (OEI) under your health benefits (who is NOT an MSU benefits-eligible employee or retiree) and want to continue their coverage during the next plan year, you MUST complete the Affidavit each Open Enrollment period between October 1-31.

Steps to Complete the Health Plan Affidavit for Spouse/OEI

1. Log into the  EBS Portal   using your MSU NetID and password. If you do not know your MSU NetID and/or password, please contact MSU IT at 517-432-6200.
2. Complete two-factor authentication. If you have not registered for two-factor authentication,  please visit this website for instructions.
3. Click the  My Benefits  tab in the top navigation.
