QUALIFYING LIFE EVENT: MARITAL STATUS
Marital Status Checklist:
Marriage
Enrolling New Dependent(s) in Benefits:
- Complete an Add Family Member or Dependent form and provide supporting documentation such as a marriage license or certificate in the
EBS Portal
(within the My Benefits section) within 30 days of marriage.
- Once forms are approved, you must enroll your eligible dependent(s) in benefits in the EBS Portal (within the My Benefits section > Benefit/Retirement tile).

Other items you may want to consider as a result of your marriage:
- Change your
Beneficiary Designation
for any life, accident insurance, or retirement policies you may have.
- Make address or phone number changes in the EBS Portal (within the My Personal Information > Personal Profile tile).
- If you are changing your name, give a copy of your new Social Security card to your department or directly to MSU Human Resources at 1407 S. Harrison Rd., Suite 110, East Lansing, MI 48823.
- To obtain a new Social Security card take appropriate documentation (marriage license, court documents) to the nearest Social Security Office.
- Contact
University Health & Wellbeing (UHW)
or the
Employee Assistance Program (EAP)
for information on services that are available for helping your child(ren) adjust to remarriage.
Legal Separation or Divorce
Removing Dependent(s)/Enrolling in Benefits:
- Submit a copy of your official legal/court documents to MSU Human Resources within 30 days of the legal separation or divorce being finalized.
- If applicable, complete a
Loss of Previous Coverage (LOPC)
form and submit to MSU Human Resources within 30 days of the legal separation or divorce being finalized.
