Qualifying Life Event: Loss of Previous Coverage
Loss of Previous Coverage Checklist:
Enrolling Yourself and/or Other Dependent(s) in Benefit Plans:
- Submit a completed Loss of Previous Coverage form to MSU Human Resources within 30 days of losing coverage.
- If applicable, complete the Add a Family Member or Dependent form through the EBS Portal and submit supporting documentation for each dependent (i.e., marriage license/certificate for spouse, birth certificate for any child, etc.) within 30 days of losing coverage.
- Once Documents are processed enroll yourself and/or dependent in benefits through EBS.
