Add Family Member or Dependent

Typically, dependent children are covered through the end of the year they turn age 26 for health and through the end of the year they turn age 23 for dental, child life, AD&D, and voluntary benefits such as vision.

Overview 

The following steps outline the process to Add a Family Member or Dependent.  

  • Complete and submit the Add a Family Member or Dependent form for each of your dependents you would like enroll in benefits. (see table below)
  • Submit required documentation, i.e., birth certificate, marriage certificate, to MSU Human Resources.
  • MSU Human Resources will review your form and documentation and individually approve your dependents to add them to your personnel record. 
  • You must complete benefit enrollment, adding your dependent(s) for coverage within the standard time period (typically 30 days.)

Completing the Add My Dependent form in the EBS portal

The Employee Details and Address sections are a view-only display of the information we have in your personnel record.

Event

  1. Click on the Event drop-down list and select the Life or Work Event. (required field)
  2. Enter or pick the Event date from the calendar picker. (required field)

  Dependent Details

  1. Click on the drop-down list and select the dependent type involved in this Life or Work event.
  2. Enter the dependent’s name details, including suffix when applicable, in the name fields. (Last Name is a required value.)
  3. Enter the dependent’s date of birth, Social Security Number/ITIN and choose the Gender from the drop-down list. (Date of Birth and gender are required values.)
  4. If the Family Member/Dependent is disabled click to check the box and the link for the Dependent Disability Certification form is made visible. (Optional)
    1. Click on the Dependent Disability Certification form to open and complete the form.
    2. Save the Certification form to attach or to provide in paper form to MSU Human Resources.

Acknowledgement Section

  1. Click to verify the form is complete and accurate. (Required)
  2. If present, click to acknowledge that a Social Security number is required for dependents aged 45 or older. (Required if present)
  3. If present, click to acknowledge you understand you must provide dependent documentation to MSU Human Resources. (Required if present)

Notes

  1. You may optionally choose to add a comment to this form that will be visible to MSU HR. To do so, enter text in the text box and click the arrow to save the note.

Attachments

You are required to submit documentation for review by MSU HR.  The most efficient way to submit supporting documentation to establish dependent eligibility is to attach it to the Add My Dependent form.  The steps below detail how to attach documentation (such as Birth Certificate or Divorce Decree) to your form. 

  1. Click to choose the attachment type from the drop-down list. 
  2. Click on the Upload button and browse to your file in the File Upload window.  Click OPEN to attach the file (PDF/JPG/PNG/DOC/TXT are all allowed).
  3. Repeat steps 9 and 10 for additional required documentation for this dependent.

Submitting the form

Once you have completed the form, you may review the information on screen to ensure accuracy and you can make changes if needed.

  1. Click Send to submit this form to MSU Human Resources.
    1. Click on Save Draft if you need to save and return to the form later (the saved draft will be located in your HR/Payroll Inbox).
    2. You can optionally save or print a copy of this form.  Click on Download PDF, in the upper right of the screen, and a PDF version of the form will open in a new tab.
  2. When you have successfully submitted a form, a Success window will appear giving you the reference number of the form.  Click on Homepage to return to the main portal page or Start New Form to complete an additional form for an additional dependent.
