health plan Affidavit for spouse/oei

Benefits-eligible employees and retirees are no longer required to complete the health plan affidavit.  However, the premium threshold requirement still applies.

Please see below for important information about enrolling or continuing coverage for a spouse/other eligible individual (OEI) in a MSU health plan.

Premium Threshold Requirement

If your spouse/OEI has access to health care coverage through their employer, they must purchase their employer's coverage if the annual employee premium cost for single-person coverage is:

$1,500 or less for the 2025 plan year

$1,850 or less for the 2026 plan year

The spouse/OEI employer's plan will be their primary coverage and the MSU plan will be their secondary coverage.

This coordination of benefits (COB) is required to enroll an eligible spouse/OEI in MSU's health plan.

If your spouse/OEI either is not eligible for another employer's health plan OR if they are eligible but the plan costs more than the annual premium threshold above, they are not required to enroll in or coodinate coverage between plans and can be enrolled in the MSU health plan as primary coverage.
