The forms found at the two links below are the most commonly used/requested Human Resources forms for administrators, managers, supervisors and HR professionals. All are either in PDF or Microsoft Word format unless otherwise noted. The PDF format requires the free Adobe Reader application be installed. EBS Forms

Many HR/Payroll forms are now located within the Enterprise Business Systems (EBS) Portal.

Remember to submit forms early to allow time to obtain all the approvals required to enter the action in the HR/Payroll system by the effective date of the action. Forms should meet payroll deadlines and allow Human Resources to verify employee eligibility for benefits no later than the effective date of the action.  The routing may differ depending on the type of form.
