Information about the Fixed-Term Faculty/Academic Staff Appointment/Reappointment Memorandum form
The Fixed-Term Faculty/Academic Staff Appointment/Reappointment Memorandum must be present when processing an Appointment/Reappointment for a fixed term appointee, and also when changing any of the employment characteristics which appear on the Memorandum by means of a Change of Status form.
Exceptions: The Memorandum is not necessary when processing a salary increase on a pay and cost redistribution request, nor when appointing/reappointing a person on contract whose contract is attached. Faculty members appointed in the Health Programs Faculty Appointment System should use the HP Faculty Appointment/Reappointment Memorandum.
Name: Must match the name on the Appointment/Reappointment form.
Deans(s)/Director(s) of: Must match the colleges listed on the Appointment/Reappointment form. Type out the full college name or the 18 character common unit code abbreviation.
Position/rank and/or title of: Must match the position/rank(s) and title(s) as they appear on the Appointment/Reappointment or the Change of Status "To" column.
Department(s)/School(s)/unit(s) of: Must match the departments listed on the Appointment/Reappointment or the Change of Status "To" column. Type out the full department name or the 18 character common code abbreviation.
Annual salary rate of $: Must match the Annual Salary rate shown on the Appointment/Reappointment or the Change of Status "To" column.
(plus eligibility for annual salary increase ...): Leave this phrase as is if eligible for an annual salary increase. If not eligible draw a line or lines through the phrase to delete it.
Percent time: Must match the percent time shown on the Appointment/Reappointment or the Change of Status "To" column.
Annual/Academic Year: Must match the appointment basis shown on the Appointment/Reappointment or the Change of Status "To" column.
Percent Time for: Percents for the six areas listed must add to 100%. For specialists the percents must be consistent with the specialist function (as stated on the most recent Specialist Position Description).

Unit administrator signature: Signature must be the name of a chairperson/director of one of the departments listed above with any initials written below. The signer must date her/his signature.
Employee signature: Signature must be the name of the employee and must be dated.
Routing: Follows the routing of the Appointment/Reappointment/Change of Status to which it is attached.
