Data Definitions (HRP1046)

The data columns used in the Employee Address and Phone Number with Emergency Contact Information report are defined on this page.

Employee Information:

  1. Professional Name:  The employee’s professional name as indicated in the HR/Payroll system.
  2. Personnel Number:  An HR/Payroll system generated number linking a person to an assignment.
  3. Organization Code-Name:  The primary college or MAU code and name in which the employee is assigned.
  4. Department Code-Name:  The primary department code and name in which the employee is assigned.
  5. Job Title:  The employee’s primary job title or classification in which the employee is assigned.
  6. Address Line 1:  The employee’s home address line 1 as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).  
  7. Address Line 2:  The employee’s home address line 2 as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  8. City:  The employee’s home city as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  9. State:  The employee’s home state as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  10. Zip Code:  The employee’s home zip code as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  11. Phone Number:  The employee’s home phone number as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their phone number.  

Emergency Contact Information:

  1. Contact First Name:  The employee’s emergency contact’s first name as indicated in the HR/Payroll system.  
  2. Contact Last Name:  The employee’s emergency contact’s last name as indicated in the HR/Payroll system.
  3. Address Line 3:  The employee’s emergency contact’s address line 1 as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  4. Address Line 4:  The employee’s emergency contact’s address line 2 as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  5. City1:  The employee’s emergency contact’s city as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  6. State1:  The employee’s emergency contact’s state as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  7. Zip Code1:  The employee’s emergency contact’s zip code as indicated in the HR/Payroll system.  RESTRICTED will be shown for employee’s who have selected to restrict their home address (reason S or B).
  8. Phone Number1:  The employee’s emergency contact’s phone number as indicated in the HR/Payroll system.
