Support Staff Policy & Procedure for Voluntary Employee-Paid Accidental Death & Dismemberment Insurance

Policy

The University provides access to an Accidental Death and Dismemberment (AD&D) plan for eligible employees.

Eligibility: All regular full-time and part-time (50% time or more) faculty and academic staff and University support staff. 

Effective date of benefit: Coverage will be effective as of the date of employment or appointment to an eligible status provided enrollment occurs within 30 days. Enrollment beyond 30 days cannot occur until the annual open enrollment period. Employees not actively at work on that day will become covered on the day they return to work.

Maximum Employee Benefit:$1,500,000

Maximum Spouse Benefit:$750,000

Maximum Child Benefit:$100,000


Premium contribution: The program is entirely funded from employee premiums and rates are subject to change. The employee's monthly contributions are shown in the chart above or at the MSU Human Resources office.

Termination date of benefit:

  1. Coverage will terminate on the last day of the month following the month in which the last deduction is taken.
  2. Employees on unpaid leave of absence, who wish to maintain coverage, must forward the full premium payment to MSU Human Resources. Coverage allowed to lapse, due to non-payment of premiums, will automatically be reinstated to the coverage in effect prior to the leave. (The employee must return to an eligible status.)
  3. Dependent coverage terminates as above. In addition, certain changes to dependent status will terminate coverage for dependents.

Conversion of coverage: No conversion is possible.

Procedure

Application for enrollment:

  1. Enroll within 30 days of initial employment or appointment to an eligible status by accessing the EBS Portal with your MSU NetID and password.
  2. If enrollment does not occur within 30 days of initial employment or appointment to an eligible status it is necessary to wait for an annual open enrollment period.

Changes in coverage or personal information: 
Employee:

  1. Change the amount of coverage:
    • may increase or decrease coverage
    • may add eligible dependents.
  2. Change of beneficiary: May change beneficiary at any time.
