Campuswide Health Screening Form

All faculty, staff, student employees and students should use the University's official health screening form. Units should use this form moving forward rather than any forms created in the unit. MSU IT and the University Physician’s office have created this form and own this information.

Do You Cover a Spouse or Other Eligible Individual (OEI) on Your Benefits?

If you cover a spouse or other eligible individual on your benefits, you must complete the Health Care Affidavit before October 31, 2020 if you wish to continue their coverage. There is not a separate Affidavit-Only option through the EBS Portal; you must follow the Open Enrollment steps here to complete the Affidavit.

Annual Security and Fire Safety Report

The Annual Security and Fire Safety Report contains information about reporting crimes, campus crime and fire statistics for the previous three years, as well as information about the university’s Relationship Violence and Sexual Misconduct and Title IX Policy. The report also contains the MSU Alcohol and Drug Policy, information on drug and alcohol educational and counseling resources, defines prohibited behavior, and outlines consequences for violations.
