Campuswide Health Screening Form

Effective immediately, all units should begin using the University's official health screening form for all faculty, staff, student employees and students. Units should use this form moving forward rather than any forms created in the unit. MSU IT and the University Physician’s office have created this form and own this information.

I-9 Reverifications

As a reminder, the I-9 reverification process is to be conducted by the unit. Please direct any employee with H1B Visa or other special visa circumstances (such as a pending STEM OPT extension) to the Office of International Students and Scholars (OISS) Room 105 at the International Center to complete their work authorization reverification with an OISS I-9 Verifier. 

 If you need assistance with the completion of the form, please contact the office of OISS at 517-353-1720. Virtual/remote I-9 processing will still available through August 18, as the temporary provisions issued by the Department of Homeland Security expire August 19. For assistance after August 18, please contact the OISS office for further guidance on how to schedule an appointment to complete the Form I-9.
