Our records indicate that you currently cover a spouse on your benefits and have not yet participated in Open Enrollment for the 2008-09 plan year.
I’m writing to remind you that in order to continue spousal coverage from year to year, employees must participate in Open Enrollment, which includes completing an affidavit certifying that your spouse is still eligible for coverage. This is true even if you aren’t planning on making any changes to your benefits plan options. The only exception is that if both spouses work at MSU, completing an affidavit is not necessary. In all other cases, employees who want to continue covering an eligible spouse on their health benefits must participate in Open Enrollment and complete the affidavit every year. If a new affidavit is not completed each year, spousal benefits may be cancelled. Though eligible spouses can be re-enrolled after the affidavit is completed, an interruption in coverage may occur.
